The UMTRC is offering scholarships to the first 100 individuals in each of our four regions (Illinois, Indiana, Michigan, Ohio) to attend the workshop at no cost. If you are not located in one of the four regions above, click below to register as a general attendee.  Ticket cost is $100. No refunds after May 7, 2021. 


DESIGNATION STATEMENT:

Union Hospital designates this live activity for a maximum of 6.0 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. 

CME ACCREDITATION STATEMENT:

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Indiana State Medical Association (ISMA) through the joint providership of Union Hospital and the Indiana Rural Health Association. Union Hospital is accredited by the ISMA to provide continuing medical education to physicians.
